Policies and Forms
Insurance
Our office is out-of-network for all major insurance plans, except for Medicare. At the time of service, our staff will provide you with a receipt and a Health Claim Form (HCFA) with all of the necessary information, including diagnosis and service codes, required by your insurance company to process your claim. The Health Claim Form will then be ready for you to mail out that same day. You will be reimbursed directly by your insurance company according to your specific policy.
Medicare Patients
If you have Medicare, we will bill them directly for you. You are only responsible for charges applied to your deductible, any co-insurance, or charges not covered by Medicare. Some Medicare secondary insurances are known as Medigap policies. With a Medigap policy, Medicare automatically forwards your claim to your secondary insurance carrier if they have a contract with the carrier. This is known as a "crossover" or "medigap." We do not directly bill all supplemental secondary insurance carriers. Therefore, if your secondary insurance does not crossover to a Medigap policy, you are responsible for that portion of your charges at the time of service (which is typically 20% of the covered charges).
For example, if a standard office visit costs $100, Medicare will cover 80% or $80. If your secondary insurance does not crossover, you will be responsible for the remaining 20% or $20 at the time of service. You may then submit your receipt to your secondary insurance in order to request reimbursement. We will help you expedite this process by providing you with a Health Claim Form with all of the necessary information and codes required by your insurance company to process the claim.
Lab-Work or Biopsies
Regardless of your insurance, if you require lab-work during your visit, you may receive a bill from the laboratory where you get your lab-work drawn. If you require a biopsy during your visit, there may be a separate bill both for the lab that processes the biopsy and the physician who reads the biopsy.
Payment
We accept MasterCard, Visa, cash, and checks. Payment is due at the time of service. All cosmetic consultation and procedure fees must be paid in full before the consult or procedure is performed. We do not bill or accept payment from insurance companies.
Cancellations
As a courtesy to all of our patients, we ask that all cancellations be made at least 24 hours in advance of the scheduled appointment time. Failure to comply with this request will lead to a $25 "No Show" fee.
Day of Appointment
In an effort to serve you better, we ask that new patients arrive 15 minutes prior to their scheduled appointment time. This time is necessary to allow for new patient registration. We ask that follow-up patients arrive for their appointments on time. If you are late, we will make every effort to see you the same day. However, you may be asked to reschedule if the doctor's schedule is full.
We value our patients' time and make every effort to stay on schedule. This is the reason why we ask that you be on time. However, please understand that it is the nature of our specialty that some patients may require additional time with the doctor for emergency or unforeseen treatment.
What to bring
To allow us to better serve you, please bring the following information to your appointment:
- Your Driver's License
- Name, fax and phone number of your primary doctor and the consulting or referring doctor if applicable.
- Name, fax and phone number of your preferred pharmacy.
- If Medicare, your insurance card and your prescription card. You cannot be seen without presenting your valid insurance card at each appointment.
- Any referral or consultation paperwork necessary.
- Your medications (including herbal supplements, vitamins and over the counter). Please bring the bottle for oral medications and the tubes or jars for any soaps, creams, shampoos or cosmetics.
- Completed and signed registration form, medical history form, and review of systems forms. This will greatly speed up your first appointment wait time.
- Credit card or other preferred method of payment.
